Referring a Patient to Metrolina Eye Associates
At Metrolina Eye, we strive to make the referral process as smooth and efficient as possible for our referring doctors. We offer three (3) convenient options to accommodate your preferences and streamline the process of sending a patient to our care. Our goal is to contact patients within 24 hours. Please be advised, Medicaid can take up to 72 hours to process for eligibility.

Required Referral Information
- Referring provider details (name, address, phone, practice, email, NPI)
- Provider’s direct fax number (for appointment confirmation)
- Patient demographics (DOB, race, ethnicity, gender, language)
- Copies of medical and vision insurance cards (front & back)
- Insurance subscriber details (if not the patient)
- PCP-issued insurance referral (if required)
- Exam notes with diagnosis and current medications
